Job Description
Administer skilled nursing care to patients in accordance with the established plan of care.
Assist in providing nursing services and identifying the patient's needs evidenced by observation and documentation.
Participate in planning, coordinating, and implementing care with the RN to provide quality patient care.
Provide effective communication to patient / family, co-workers, and other health care professionals.
Assist the patient / family in learning appropriate self-care techniques.
Utilize infection control measures that protect both staff and patient.
Assist the RN and / or physician in performing specialized duties related to end-of-life care for Hospice patients.
Critical Information
Schedule notes indicate approximately 38 visits with no on-call duties.
Territory will be west and southwest.
Education / Licenses Needed
Current Nursing License in the Practicing State.
Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
Current CPR Certification.
Two years experience as an LPN / LVN in a clinical setting, preferably in home health or hospice.
