Location : Grand Junction, CO

Pay : $48–$52 / hour (DOE) + $5,000 Signing Bonus for Full-Time Roles

Schedule : Flexible, autonomous daytime hours

In this role, you'll have the freedom to manage your schedule, deliver individualized care, and help patients regain independence in the environment they know best-their home.

What You'll Do

As a Home Health Occupational Therapist, you will :

Provide personalized, in-home therapy services to a diverse range of patients.

Assess functional limitations and develop customized treatment plans.

Conduct home safety evaluations and recommend adaptive equipment and modifications.

Collaborate closely with nurses, PTs, caregivers, and family members to ensure coordinated, holistic care.

Track and document patient progress accurately (training provided for EMR system).

Travel locally throughout Mesa County (mileage and drive time reimbursed).

Who We're Looking For

Licensed Occupational Therapist (OTR) in the State of Colorado (required).

Home health experience a plus!!

Excellent communication, organization, and time-management skills.

Passion for empowering patients to live safely and independently at home.

Valid driver's license, reliable vehicle, and current auto insurance.
