• Verify plan of care with the RN prior to each visit.\n
• Collaborate with physician and RN frequently to ensure patient needs are being met; Notify them as soon as possible when patient condition changes.\n
• Teach patients and caregivers self-care techniques.\n
• Check vital signs.\n
• Review all medications and perform a medication reconciliation at each visit.\n
• Perform specialized procedures ordered by physician (i.e. foley catheter change).\n
• Prepare equipment and materials for treatments using the aseptic technique.\n
• Complete documentation timely and accurately.\n\nQualifications\n
• One year of clinical LPN experience.\n
• Current LPN license, specific to the state(s) you are assigned to work.\n
• Current CPR certification.\n
