Job descriptionA t MFM Health, we are committed to your professional development. Monitor and respond to Bamboo Patient Ping with appropriate next steps regarding patient inpatient admissions. Notification of admit status also via clinical documents in Athena as well as Epic. Performs TCM process including post d / c calls within appropriate time frame as well as scheduling HFU and ensure that visit is prepped accordingly with TCM outreach information. Provide CM outreach and appropriate workflows to patients enrolled in CM programs such as CCM / APCM. Complete Tuck-In Calls to patients enrolled in Care Management programs. Care Manager MFM Health in Danvers, MA is seeking a Care Manager to support high-risk patients by helping them set health goals, address barriers to care, and promote self-management.
Collaborating with patients, families, and the healthcare team, they coordinate care, develop and implement care plans, monitor progress, and encourage treatment adherence.
