This role is part of Grace Works Academy and focuses on employment mentorship and vocational development, not residential or personal care services.

Job Description

Grace Reliant's growing Grace Works Academy team is looking to invite a Consumer Service Provider to help individuals with disabilities and other barriers build skills for competitive employment and community inclusion. We exist to help people live more complete and independent lives by teaching, mentoring, and guiding individuals toward meaningful work opportunities in Missouri.

In this role, you will work closely with consumers to identify and augment their skills, interests and abilities using individual and / or group experiences. These skills, interests and abilities inform the job exploration and job development activities to follow with the ultimate goal of opening a career path for each consumer.

Job Responsibilities

• This CSP position focuses on teaching and mentoring individuals, not personal care or home-based supports
• Engage with consumers to build employment readiness and follow interventions and objectives identified in their Specific Service Plan (SSP)
• Facilitate consumer group experiences as assigned
• Document every consumer engagement using Person-first, professional language per agency and legal requirement
• Transport consumers as needed and assigned
• Collaborate with Grace Works Academy staff to advocate and help plan future goals for consumers
• Other duties as assigned
