Pay Information $2,346 per week About The Position Job Title : Hospice Nursing Case Manager Profession : Registered Nurse Specialty : Case Manager Duration : 13 weeks Shift : Day Hours per Shift : 8 hours Experience : Must have 1 year of RN Hospice Case Management experience. License : Active RN license required. Certifications : Certifications required for allied positions. Must-Have : - Active driver's license. - EPIC charting experience. Description : Hospice RN Case Manager needed for a hospice team. Schedule is Monday through Friday, with every third weekend rotation. Hours range from 8 : 00 AM to 4 : 30 PM or 9 : 30 AM to 5 : 30 PM. The case management position involves managing a caseload of 15-18 patients. Patients are located in private homes and facilities. The position requires assessments, pain and symptom management, and coordination of care. The team meets for interdisciplinary team meetings once a week. Visits and admissions are measured in points, with specific point values assigned for each type of visit or event. An emphasis on providing compassionate care during the end-of-life journey is vital. Candidates must demonstrate a strong understanding of hospice care protocols. Excellent communication and organizational skills are essential for success in this role.
