License : Active RN license required. Certifications : Certifications required for allied positions. Must-Have : - Active driver's license. - EPIC charting experience. Description : Hospice RN Case Manager needed for a hospice team. Schedule is Monday through Friday, with every third weekend rotation. Hours range from 8 : 00 AM to 4 : 30 PM or 9 : 30 AM to 5 : 30 PM. The case management position involves managing a caseload of 15-18 patients. Patients are located in private homes and facilities. The position requires assessments, pain and symptom management, and coordination of care. The team meets for interdisciplinary team meetings once a week.
