Are you passionate about providing psychosocial support to patients and their families during challenging times? We are seeking a dedicated Social Worker (MSW) to be a part of our interdisciplinary team at Gentiva Hospice.

Your Role :

The Social Worker (MSW) plays a vital role in delivering comprehensive psychosocial support by assessing the unique needs of patients and their families. You will develop personalized care plans, ensuring that emotional, social, and practical needs are met with compassion and understanding.

About You :

• Master's Degree in Social Work from a CSWE accredited School
• Valid state licensure as required for hospice practice
• At least one year of experience as a Social Worker in healthcare and / or hospice settings; three to five years preferred
• Strong knowledge of the needs of terminally ill patients and their families, with sensitivity to the psychosocial dynamics of illness, loss, and death
• Ability to provide support to individuals during their most challenging moments, while managing the stress of repeated loss
