Job description
A company is looking for a Nurse Practitioner to support patients in their remote care journey.
Key Responsibilities
Oversee monitoring of patient vitals, symptoms, and labs in collaboration with RNs and MDs
Manage treatment plans and medications, focusing on guideline-directed medical therapy for chronic conditions
Conduct regular virtual check-ins with patients to optimize treatment adherence and review lab results
Required Qualifications
5+ years of experience treating chronic diseases (T2D, Hypertension, CHF) in a medical setting
Active multi-state compact RN license and Nurse Practitioner (NP) license in Texas and additional states
ANCC or AANP certification required
Master's Degree as a Nurse Practitioner with willingness to expand state licensure
Experience managing CHF with guideline-directed medical therapy preferred
