Key Responsibilities Obtain and share information on benefits and community resources for patients post-discharge Assist in processing Medicaid applications and other necessary documentation for patient benefits Collaborate with nurse coaches to provide patients with information on community resources for recovery support Required Qualifications, Training, and Education Degree in social work or a related field Deep knowledge of community amenities and resources Strong understanding of medical terminology and planning Experience in recognizing Health Plan benefits and community resource needs 5+ years of experience in community resource management or Health Plan benefit management
