About The Position

Job Title : Hospice RN, Home Health RN

Profession : Nursing

Hours per Shift : 8am-4pm

Experience : Minimum 2 Years

License : Active RN License

Certifications : CPR Certification

Must-Have :

  • Minimum 2 years of nursing experience
  • Experience with OASIS documentation
  • Knowledge of evidence-based practice
  • Effective communication and case management skills

Description :

The role requires providing hospice care and home health services.

You will effectively complete OASIS documentation in accordance with policies and regulations.

Demonstrate knowledge of OASIS documentation requirements and ensure timely completion.

Utilize the nursing process to assess patient and caregiver needs to provide quality care.

Develop and communicate a realistic plan of care for each patient.

Ensure patient and caregivers are included in the care plan process.

Serves as a case manager for assigned patients while managing caseloads and visit times.

Implement the medical plan of care and nursing interventions as necessary.

Stay updated on high-tech skills such as IV use and maintenance, wound VAC, and complex wound care.

Ensure infection control standards are maintained throughout the plan of care.

Collaborate with LPNs to ensure that the plan of care is followed.

Delegate tasks and ensure that patient needs are met.

Review documentation and act on recommendations per department policies.

Evaluate the effectiveness of the implemented plan of care.

Maintain up-to-date and accurate documentation of nursing care provided.

Follow agency protocols with documentation processes.

Educate patients and caregivers on medication side effects and treatment options.

Identify interdisciplinary needs of the patient and document accordingly.

Fill out required agency forms in a timely manner.

Carry out visits efficiently and communicate information to the scheduler promptly.

Assess patients' needs for services and coordinate plans of care.

Transmit patient incident reports to the quality improvement specialist as needed.

Coordinate safe patient discharges or transfers, processing all necessary paperwork.

Facilitate communication and care coordination with all related parties.

Participate in monthly peer review audits, providing and receiving feedback professionally.

Schedule and facilitate case conferences with team members to ensure effective communication.

Advocate for patient care issues to ensure sensitivity to specific patient and family needs.
