Position Description

Jubilee Housing operates a continuum of housing and services for men and women returning to the community from incarceration, beginning with the earliest days home in transitional and SRO housing, through long-term, service enriched affordable housing.

The Department of Reentry Housing and Services is seeking a dedicated and compassionate Reentry Service Coordinator to support individuals who are ready for long term housing in a service enriched environment. The Coordinator will be responsible for 33 households across two affordable housing communities in the long-term service enriched part of the program. The Coordinator will be involved screening and selecting residents as well as accompanying them during their ongoing pursuit of life goals. The role will also provide support and leadership to two housing communities, nurturing a sense of belonging and purpose in a peer supported community. The ideal candidate will be skilled in providing resources, advocacy, and guidance to ensure successful long-term reintegration into society.

Specific Duties

• Screening and Selecting : Lead interview process for new residents, interpreting mission and vision of the program and determining readiness and suitability for the housing community.
• Assessment and Planning : Conduct individual assessments to identify the needs and barriers of clients reentering the community. Develop personalized reentry plans that address housing, education and career, family reunification, mental health, and recovery needs.
• Resource Coordination : Connect clients with local resources, such as homeownership opportunities employment training programs, healthcare providers, and educational opportunities. Maintain a comprehensive database of community resources.
• Advocacy : Serve as an advocate for clients, assisting them in navigating the complexities of public services and legal requirements related to their reentry process.
• Case Management : Monitor client progress through regular follow-ups and check-ins. Adjust reentry plans as needed and provide ongoing support and encouragement.
• Collaboration : Work collaboratively with various stakeholders, including correctional facilities, social service agencies, law enforcement, and community organizations to facilitate a smooth transition for clients.
• Education and Support : Conduct workshops and seminars to educate clients on their rights, responsibilities, and available resources. Provide emotional support and motivation to empower individuals during their reentry journey.
• Documentation : Maintain accurate records of client interactions, services provided, and outcomes. Prepare reports for internal use and funding agencies as needed.
