Physical Therapist (PT) – Home Health- $7,000 Sign-On Bonus Location : Sun City, AZ Work Type : Full-Time Compensation : PPV (Per-Visit Pay Structure) About the Opportunity A respected and patient-centered home health organization is seeking an experienced Physical Therapist to join its growing clinical team serving Arizona’s West Valley region. This position offers the chance to provide meaningful, one-on-one care in the comfort of patients’ homes, helping them improve mobility, regain independence, and enhance their quality of life. As a Physical Therapist in home health, you will conduct evaluations, develop customized treatment plans, and guide patients through rehabilitation tailored to their functional needs. You’ll collaborate closely with an interdisciplinary care team to ensure coordinated, high-quality service delivery. Key Responsibilities Perform comprehensive evaluations of patients’ physical condition, mobility, and functional limitations Develop and implement individualized physical therapy treatment plans Provide patient and caregiver education on exercise programs, safety practices, and rehabilitation techniques Maintain thorough and timely documentation in accordance with organizational and regulatory standards Coordinate with nursing, occupational therapy, and other disciplines to ensure seamless, effective care Travel throughout the West Valley territory to conduct in-home patient visits Required Skills Active Physical Therapist license in Arizona Doctorate or Master’s degree in Physical Therapy Minimum 1 year of experience as a Physical Therapist Home health experience strongly preferred Current BLS / hands-on CPR certification Reliable transportation with a valid driver’s license and auto insurance Additional Skills to Consider Experience working within EMR systems Understanding of Medicare home health guidelines Experience managing a home-based therapy caseload Proficiency in creating functional, goal-oriented care plans Benefits Include Medical, Dental, and Vision Insurance Paid Time Off (vacation, sick leave, holidays) Mileage reimbursement Employee Assistance Program (EAP) Health Savings & Flexible Spending Accounts Disability, Life, and AD&D Insurance 401(k) retirement plan Why This Opportunity Stands Out This position offers the autonomy and flexibility of home health paired with the support of a coordinated clinical team.
