What you will be doing :

• Act as a critical part of the care team, providing advanced clinical assessment and developing individualized care plans based on patient needs.
• Conduct thorough patient health histories via direct interviews with patients and / or their families.
• Utilize specialized knowledge and skills to deliver effective care, identify potential diagnoses, and create targeted interventions.
• Maintain accurate, up-to-date patient records and documentation.
• Promptly respond to patient inquiries, ensure follow-up on referrals, and provide consistent communication.
• Occasionally serve as the designated on-call clinician to address patient emergencies during scheduled work hours.
• Collaborate closely with multidisciplinary teams, including primary care providers, specialists, psychiatrists, and community programs, to ensure comprehensive behavioural healthcare for patients of all ages.
