What You Will Do
- Provides psychosocial assessment / intervention to assist patients / families, (including clinically complex).
- Communicates / collaborates with the work team using a multidisciplinary approach.
- Provides linkage to community resources to facilitate optimal care.
- Provides age specific care to patients / families in all age groups.
- Develops and implements a comprehensive plan based on identified psychosocial needs and / or discharge needs.
- Maintains a current working knowledge of UH network services and promotes utilization of such services.
- Provides guidance and serves as consultant to staff and other disciplines regarding complex psychosocial and / or discharge planning issues / barriers.
- Provides supervision to social work interns if LPCC eligible (completion of supervision hours).

Master's Degree from accredited school of Counseling (Required)Work Experience

Attainment of LPCC status is encouraged with one year of eligibility. Licensed Professional Clinical Counselor (LPCC) (Required)LPCC-S (Preferred)Position Details / Preferred Qualifications
This outpatient therapist will be seeing patients with various mood disorders, including depression, anxiety, bipolar, etc. Previous experience working with this patient population is strongly prefered. Must be proficient in performing diagnostic assessments, treatment planning, documentation, etc. Having an LPCC-S is preferred. Knowledge of Epic is a plus.
