Job Title : Speech Therapist

The Speech therapist develops and implements a plan for providing rehabilitation home treatment as

well as educating the patients and family members. They will also coordinate with other HH staff to

provide integration of care if needed.

Speech Therapist Responsibilities / Roles / Skills

• Evaluate patients' communication strengths and weakness
• Communicate with the patient, family and physician plan of care
• Evaluate a patients progress and implement changes when needed
• Ability to organize, use time efficiently and communicate well
• Plan and / or assist with patient discharge from home health speech services
