Description :
Utilizing the nursing process, assesses patient and caregiver needs to provide quality care to assigned patients.
Develops and communicates a realistic plan of care for each patient involving patients and caregivers in the process.
Serves as case manager for assigned patients and manages caseloads and visit times in accordance with productivity standards.
Effectively completes OASIS documentation in accordance with policies and regulations.
Implements medical plans of care, nursing interventions, and procedures to ensure safe, effective quality of care.
Stays up-to-date on high-tech skills such as IV use and maintenance, wound VAC, and complex wound care.
Ensures infection control standards are maintained throughout the plan of care.
Works collaboratively with LPNs to follow the plan of care, delegates tasks, and ensures patient needs are met.
Evaluates various aspects of the implemented plan of care, including identified problems and patient responses.
Maintains accurate documentation of nursing care provided for integration of information among the health care team.
Follows agency protocol with documentation processes including locking-down visits, follow-up notes, and visit completion.
Performs education for patients and caregivers regarding medication side effects, treatment options, and discharge instructions.
Identifies interdisciplinary needs of the patient and documents accordingly, communicating information to other disciplines.
Accurately fills out a variety of agency and regulations forms in a timely manner.
Carries out visits efficiently, ensuring supervisory visits are not combined with routine visits when necessary.
Assesses patient needs for home care assistance services and develops a plan of care with the patient and caregiver.
Transmits patient incident reports to Quality Improvement Specialist promptly.
Coordinates safe patient discharges and transfers, ensuring all paperwork and patient information is processed accordingly.
Coordinates a variety of patient care with physicians, providers, and team members to effectively support the plan of care.
Participates in monthly peer review audits, providing and accepting constructive feedback promptly.
Schedules and facilitates case conferences with team members to enhance communication.
Advocates for patient care issues to ensure quality of care sensitive to patient and family needs.
Carries out comprehensive assessments of patient and caregiver needs for quality care provision.
Actively participates in interdisciplinary team meetings to develop an individualized plan of care.
Stays informed on pain control, symptom management, and infection control standards.
Performs patient and caregiver education to support informed decision-making.
Coordinates care with physicians, providers, and vendors to ensure safe and effective patient and family care.
