Job descriptionProvide patient care and lead a collaborative healthcare team. Manage both acute and chronic conditions. Promote wellness, education, and prevention. We are seeking a full-time Advanced Practice Provider (APN / PA) or MD for the role of Health Center Medical Director (HCMD). Family medicine providers preferred. Bilingual (English / Spanish or English / Polish) a plus.
