Travel Physical Therapist (PT) – Long Term Care (LTC)

Location : Groveton, TX

Start Date : 10 / 27 / 2025 | Duration : 13 Weeks

Shift : Days | 8-hour shifts | 40 hours / week guaranteed

Weekends : None | On Call : None

Position Summary :

The Physical Therapist will evaluate and treat patients to restore mobility, reduce pain, and prevent disability in a long-term care setting. They will develop and implement individualized treatment plans aimed at maximizing patient independence and quality of life.

Essential Responsibilities :

Perform comprehensive evaluations, including history, systems review, and tests / measures.

Develop individualized plans of care based on findings and physician orders.

Reassess and modify treatment as needed based on patient progress.

Manage discharge planning from the first visit through completion of care.

Supervise and delegate appropriately per state regulations.

Complete clear, timely documentation in compliance with facility and insurance standards.

Communicate effectively with patients, families, and the healthcare team.

Provide patient and caregiver education as needed.

Participate in professional development and quality improvement initiatives.

Maintain safety standards and adhere to all regulatory requirements.

Requirements :

Minimum 2 years of Physical Therapy experience (Required)

Active TX PT License (Required prior to start)

BLS certification (Preferred)
