Job Summary :
The Director of Care Transition (DCT) is responsible for developing, coordinating, and overseeing the facility's marketing and public relations programs to ensure optimal occupancy and enhance facility awareness of our services. This role involves planning and evaluating marketing strategies, building relationships with community stakeholders, and managing media relations. The DCT will also lead any discharges needed.
Essential Job Functions :
Basic Duties
- Develop and lead the facility's marketing and public relations strategies to maintain and increase census levels.
- Create and implement marketing campaigns and community outreach programs to promote the facility's services, programs, and practices.
- Conduct market research to identify opportunities for growth and community engagement.
- Represent the facility to external stakeholders including physicians, discharge planners, local businesses, community organizations, and educational institutions.
- Conducts assessments, facility tours and pre-admission interviews with prospective residents and their families, explaining services and admission procedures.
- Move in paperwork, gift baskets and getting resident oriented in the facility on move in day.
- Facilitate move outs and move outs paperwork
- Build and maintain relationships with community figures and organizations to enhance the facility's visibility and reputation.
- Maintain proactive communication with media outlets, including newspapers, radio, and television, to publicize facility news, programs, and promotions. Develop and regularly update a database of media contacts.
- Available on weekends for assessments and tours.
- Monitor and report on media coverage and noteworthy items that could impact the facility or its regulatory environment.
- Keep the Administrator informed about significant media coverage and developments that could affect the facility.
- Collaborate with facility leadership to ensure alignment between marketing efforts and organizational goals.
- Attends a stand up with Administrator each week to discuss duties, goals etc.
- Performs other related duties as required or requested by Admin.
Risk management, Safety and Sanitation
- Is knowledgeable of and complies with company policy and procedures.
- Must report on-the-job injuries to supervisor within 24 hours of incident.
- Must be able to assist in the evacuation of residents.
- Must be able to follow emergency, fire, and safety procedures.
- Assists with building security while on duty.
- Ensures that all personnel performing tasks involving potential exposure to blood / bodily fluids participate in in-service training.
- Follows infection control and universal precautions policies / procedures to assure that sanitary environment is always maintained.
ADDITIONAL DUTIES
- Displays professionalism and integrity by adhering to the communities' code of conduct.
- Must promote the community in a positive way.
- Must maintain confidentiality of verbal and written information pertaining to residents, facility operations, and personnel.
- Must understand, follow, and help enforce the communities' policies including harassment and complaint procedures.
- Must maintain good personal hygiene (neat, clean, and well-groomed appearance).
- Must adhere to dress code.
- Must practice dependable, regular attendance.
- Must not work under the influence of mood-altering or illegal drugs or alcohol.

Customer Service
- Must demonstrate the ability to communicate well with family, residents, staff, and management.
- Must be able to communicate with medical staff, nursing personnel, and other departments.
- Must maintain a professional attitude and establish constructive working relationships with co-workers, residents, families, visitors, and public.

Resident Rights
- Ensures that resident's rights are protected and well-respected as required by the community and RCFE regulations.
- Understands role as a mandated reporter of suspected dependent adult / elder abuse and to report such incidences using Form SOC 341, as provided in employee packet as well as unusual incident / Injury report LIC 624.
- Must maintain confidentiality of verbal and written information pertaining to residents, facility operations and personnel.
WORKING CONDITIONS
- Works in office area(s) as well as throughout the community (i.e., medication rooms, common areas, resident rooms, etc.).
- Moves intermittently during working hours.
- Is subject to frequent interruptions.
- Is involved with residents, family members, personnel, visitors, government agencies / personnel, etc. under all conditions and circumstances.
- Is subject to hostile and emotionally, upset residents, family members, personnel, visitors, etc.
- May work beyond normal working hours even on weekends and holidays, and on other shifts / positions when necessary.
- Is subject to call back during emergency conditions (I.e., evacuation, post-disaster).
- Attends and participates in continuing education programs and in-services.
- Is subject to injury from falls, burns from equipment, odors, etc. throughout the day, as well as to reactions from dust, disinfectants, and other air contaminants.
- Is subject to exposure to infectious waste, diseases, conditions, etc., including TB, AIDS, and Hepatitis B.
EDUCATION AND WORK EXPERIENCE REQUIREMENTS
- Must be a minimum of 18 years of age.
- Degree in marketing, communications, or a related field preferred.
- Must show expertise in effective communication principles, mass media, advertising, and community relations relevant to Assisted Living facility operations.
- Proven ability to conduct direct sales calls, organize, public presentations and to assess referrals and work with families.
- Relationships with Hospitals, Skilled Nursing, ALW and Health Plans
- Must be able to read, write, speak, and understand the English language.
- Must possess the ability to understand and carry out both written and verbal directions.
- Must have organizational skills to manage multiple tasks, prioritize effectively, and maintain organized documentation.
- Must possess the ability to exercise sound judgment and make independent decisions when circumstances warrant it.
- Must have the ability to implement the communities' programs, goals, objectives, and policies and to interact professionally with residents, families, staff, and the public, and handle challenging situations with respect.
- Must be knowledgeable of federal and state regulations regarding the operations of a Residential Care Facility for the Elderly).
- Must possess a genuine Interest and concern for geriatric and disabled people.
- Must possess a valid driver's license and clean DMV record.
- Must receive a criminal record clearance from the US Department of Justice and the FBI.
HEALTH REQUIREMENTS
- Must be in good general health and demonstrate emotional stability.
- Must be able to cope with the mental and emotional stress of the position.
- Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the Community.
- Must undergo a health screening, including a chest x-ray or TB to test for tuberculosis, (must be free of communicable disease-) performed by a physician not more than six (6) months prior to or seven (7) days after employment.
