You will report directly to the Chief Nursing Officer or Administrator on Call.\n\nKey Responsibilities : \n
- Patient Assessment : Conduct thorough evaluations of patients' health, social support, and funding resources to develop tailored care plans.\n
- Care Coordination : Partner with doctors, therapists, and nursing teams to ensure that all patients receive the treatments and interventions they need.\n
- Discharge Planning : Create and execute discharge plans that arrange for follow-up care, home health services, or rehabilitation as appropriate.\n
- Utilization Review : Ensure the appropriateness of hospital admissions and lengths of stay, ensuring adherence to healthcare regulations while optimizing resource use.\n
- Patient Advocacy and Education : Stand up for patients' needs and provide comprehensive education on their treatment options to foster informed decision-making.\n\nSkills and Qualifications : \n
- Clinical Expertise : Solid clinical foundation in nursing and patient care.\n
- Communication Skills : Strong ability to communicate effectively with patients, families, and healthcare professionals.\n

- Basic Life Support certification within 30 days of hire.\n
