Overview We are seeking a highly motivated Licensed Nurse Care Coordinator to provide telephonic support and care coordination for patients within our facility. This full-time, onsite position in Texas requires a compassionate professional with clinic or home health experience, committed to enhancing patient engagement and health management. Required Skills - Valid Texas LVN/LPN license (verified through Nursys)
- Proven experience in clinic or home health settings
- Strong knowledge of HEDIS, CMS, and Care Gap programs
- Proficiency with Epic EMR and Microsoft Office, especially Excel
- Excellent telephonic communication and customer service skills
- Ability to work effectively with the elder population
- Familiarity with care coordination and patient engagement strategies
Nice to Have Skills - Experience working within primary care clinics
- Knowledge of healthcare quality improvement initiatives
- Bilingual communication skills
- Experience with telehealth platforms and tools
Preferred Education and Experience - High School Diploma or equivalent required; relevant nursing certification needed
- Prior experience in care coordination or case management preferred
- Minimum 1-2 years of clinic or home health experience in a telephonic role
Other Requirements - Must be able to work onsite in Texas only
- Willingness to travel or receive reimbursements if applicable
- Certification in care coordination or related fields is a plus
