We are actively seeking Full-Time Physical Therapists (PTs) to join our growing team. If you have vestibular rehab experience or are certified in lymphatic drainage , we strongly encourage you to apply—your expertise will be highly valued in our home health setting.

Position Summary :

As a Home Health Physical Therapist at Deer Meadows, you will deliver individualized therapy services in patients’ homes. You’ll play a key role in helping patients recover from illness, injury, or surgery—restoring function and confidence where they feel most comfortable. All documentation is completed using the Net Health EMR system, supporting accurate and efficient care planning and compliance.

Coverage Areas :

Northwest Philadelphia

Northeast Philadelphia

Montgomery County

Bucks County

Key Responsibilities :

Perform in-home physical therapy evaluations and develop patient-centered care plans

Deliver skilled, evidence-based therapy to improve mobility, strength, function, and safety

Educate patients and caregivers on exercises, equipment use, and fall prevention

Document all visits and progress notes accurately using Net Health EMR

Collaborate with the broader care team, including nurses, OTs, and physicians

Ensure therapy services comply with regulatory standards and agency policies

Preferred Skills & Specialties :

Vestibular rehab experience – highly preferred

Lymphatic drainage certification – strong bonus

Qualifications :

Graduate of an accredited Physical Therapy program (DPT or equivalent)

Current, unrestricted Pennsylvania PT license

CPR certification (or willingness to obtain)

Valid driver’s license and reliable transportation

Previous home health experience preferred; new grads welcome

Proficiency with electronic documentation; Net Health EMR experience is a plus

Strong communication, time management, and clinical reasoning skills
