We're a private, not-for-profit organization coordinating public health grants for a 24-county region in central PA, seeking a TEMPORARY SOR Case Manager for a short-term onsite position at our Camp Hill, PA office . This temporary opportunity is anticipated to run through December 2025 .
Requirements
The successf ul candidate will conduct outreach to individuals with opioid use disorder (OUD), including those who are unhoused or at risk of homelessness, assessing their housing needs, identifying appro priate resources and referrals , and coordinating their housing plans. They will assist clients in navigating housing systems, including applications for emergency housing, supportive housing, or transitional living programs, and work collaboratively with community partners, treatment providers, shelters, and housing agencies to secure appropriate resources for clients . This involves maintaining contact with clients through field-based work and phone check-ins and documenting all client interactions and service outcomes in accordance with grant and agency requirements . This role must ensure compliance with grant guidelines, including data tracking and timely reporting, and participation in team meetings / meetings with the team supervisor as needed.
This is a temporary onsite position with an hourly pay of $30 , depending on experience .
Bachelor's degree in a related field and Case Management , Care Coordination or Community Health Worker experience is required . Experience working with historically marginalized populations and advancing health equity is preferred. This position works with a diverse network of partners and clients; the candidate m ust possess excellent analytical, customer-service, communication skills . Must demonstrate strong competency with Microsoft Office suite.
