The PT will assess patients’ functional abilities, develop individualized treatment plans, restore function, reduce pain, and prevent disability. The therapist will work collaboratively with patients, families, and the interdisciplinary team to maximize outcomes and maintain mobility. Essential Functions and Responsibilities Perform patient evaluations including history, systems review, and relevant tests and measures. Develop individualized plans of care and select appropriate treatment techniques per physician orders. Conduct ongoing reassessment of patient progress and response to treatment. Initiate and manage discharge planning from the first visit onward. Supervise and delegate to ancillary staff and students according to state laws and facility policy. Complete accurate, timely, and legible documentation in compliance with departmental, accreditation, and insurance requirements. Educate patients, families, and caregivers to support recovery and functional goals. Participate in professional growth, in-service training, workshops, and current literature review. Actively engage in quality improvement activities. Ensure a safe environment for patients, visitors, and staff, adhering to approved safety standards.
