Experience in Skilled Nursing Facilities preferred.

Description :

Make sure the prescreen is filled out and attached upon submitting the candidate.

Resume and license verification also need to be attached.

County Criminal check (7 years for all counties lived and worked) is necessary.

Federal Criminal check (7 years or more) is required.

Multi-State Criminal Search (7 years or more) is essential.

HHS / OIG Search is needed.

GSA Search is necessary.

Social Security Number Trace is essential.

2 Step PPD or Quantiferon or proof of positive PPD and chest X-Ray is needed.

SAM Search is required.
