Full Time - Case Manager

Mahajan Therapeutics is looking for hard-working and conscientious Case Manager to fulfill primary job function of enhancing the overall treatment process by assisting therapists to meet the overall treatment needs of clients. Performs other duties as assigned. Works under the supervision of the Case Manager Supervisor and Clinical Director.

Essential Duties & Responsibilities :

• Act as a liaison between therapist, the referring agency, and family of the client.  Arrange family visits, coordinate transportation, and ensure clients’ needs are met.
• Insure communication between all components of treatment.
• Attend court hearings as deemed appropriate.
• Ensure documentation occurs according to policy.
• Attend treatment staffing with prepared report that accurately represents client’s response to treatment.
• Facilitate individual and group CSP as assigned.
• Function on agency quality assurance or other committees as assigned.
• Assist with in-service training as directed by Clinical Director.
• Attend supervisory sessions with prepared agenda.
• Ongoing assessment of client needs.
• Assistance in achieving personal independence in managing basis needs as identified by the client and / or parent or guardian.
• Facilitation of further development of daily living skills, if identified by the client and / or parent or guardian.
• Coordination of the ISP / ITP, including :

services identified in the ISP
