Field Registered Nurse Home Care

A reputable and growing home care agency is seeking a compassionate and motivated Field Registered Nurse to provide high-quality clinical care to patients in their homes. This role offers autonomy, meaningful 1 : 1 patient interaction, and the opportunity to make a direct impact on patient health and independence.

• Key Responsibilities
• Provide skilled nursing visits in the patients home, including assessments, wound care, medication management, and disease education
• Develop and update individualized patient care plans based on clinical findings and physician orders
• Document visits accurately and in a timely manner according to regulatory and agency standards
• Teach and support patients and families on health conditions, medications, and care techniques
• Communicate regularly with physicians, caregivers, and the interdisciplinary team to ensure continuity of care
• Monitor patient progress and promptly report changes in condition
• Participate in case conferences and quality improvement activities as needed
• Requirements
• Active RN license in New York
• Experience in home care, medical-surgical nursing, hospital bedside nursing, or related setting preferred
• Strong clinical assessment skills and comfort working independently in the field
• Reliable transportation and ability to travel locally for home visits
• Excellent communication, time management, and documentation skills
