Skilled Clinical Experts LLC is seeking a full-time Nurse Practitioner (np) to join our rapidly growing remote / in-home chronic care management practice. As an SCE field provider, you will treat and manage patients with multiple chronic disorders and / or transitioning from a hospital / skilled facility to the home setting. Care is provided both in-person, in the patient’s place of residence and via audio / video. Experience working remotely and independently is recommended with experience in geriatrics preferred.
