AREA OF RESPONSIBILITY
A. Total Parenteral Nutrition (TPN)
B. Chemotherapy Products
1. Obtain chemotherapy orders for inpatients and outpatients on a timely manner in order to prepare without delay.
2. Prepare chemotherapy preparation in compliance with all applicable health system policy and procedure.
3. Handle outpatient’s and investigational prescriptions according to the laws and regulations implemented by the State and our hospital regulations.
4. Properly handle chemotherapy agents using safety precautions according to set guidelines (gloves, gown, mask, proper use of laminar flow hood, etc.)
5. Know the usage of pharmacy computer in order to generate labels for medication.
6. Ensure that all chemotherapy preparations are delivered to the appropriate area on a timely basis.
7. Communicate medication order clarification with other pharmacy staff.
8. Perform other duties as assigned by supervisors.
C. Other IV Products
1. Prepare large and small parenteral solutions where there are no commercially available products.
2. Responsible for antimicrobial and other agents dosed according to patient’s weight.
3. Compound oral suspension or solution for pediatric or geriatric patients who cannot swallow whole tablets.
4. Ensures compliance with USP
Standards
5. Ensues Certification of the Laminar Flow Hood
6. Ensure appropriate training and testing of personnel perform IV compounding
7. Maintains Appropriate Records as per USP
guidelines
8. Maintain appropriate records of batch preparations and other IV admixtures
D. Distributive / Clinical Functions :
1. Report any adverse drug reaction (ADRs).
2. Provide educational lectures to pharmacy and nursing staff as needed.
3. Supervise and direct support personnel (pharmacist, pharmacy technicians, students.)
4. Verify the daily activities of pharmacist
E. Other Responsibilities :
1.Supervise and direct pharmacy support personnel.
2. Verify the daily activities of pharmacy technicians.
3.Participate in the performance appraisal of pharmacy support personnel.
4. Work independently as per professional standards.
5.Organize and prioritize work assignments.
6. Ensure pharmacy services are provided in a timely manner.
7.Identify and replace outdated and unusable drugs.
8. Answer the telephone, identifying self and department. Direct calls to appropriate personnel. Answers requests at the
window.
9. Keep pharmacy areas and equipment clean, neat, and well-organized.
10. Rotate to all areas as needed by the department.
11. Participate in multidisciplinary rounds.
12. Participate in patient care counseling activities.
13. Knows how to reconstitute, prepare, store and administer Covid-19 Vaccines
Qualifications : Qualifications :
For Level I :
1. Possession of a valid NY State license to practice pharmacy.
For Level II : In addition to meeting the qualification requirements of the lower level :
1. Two years of post licensure experience; or
2. Completion of a post graduate specialty residency or fellowship program; or
3. Current Pharmacy Board Certification.
For Level III : In addition to meeting the qualification requirements of the lower levels, an additional :
1. One year of post licensure supervisory experience or specialty clinical experience.
