Job Description

The primary function of this role is to provide Personal Care Assistance - or Direct Support and related services to the individual in their home. These tasks include :

• Assisting with household chores ( cleaning, changing linens, trash disposal)
• Assisting with bathing and hygiene
• Assisting with toileting
• Assisting with transfers, walking, exercise, or ambulation, if needed
• Assisting with activities of daily living( personal grooming, and dressing)
• Assisting with grocery shopping
• Assisting with preparing meals and snacks
• Assistance with reminders and monitoring medication
• Assist with scheduling appointments

MONDAY-FRIDAY (4 : 00 PM- 8 : 00 PM )

MUST HAVE A VALID'S DRIVERS LICENSE AND WILLING TO DRIVE

MUST BE WILLING TO WORK WEEKDAY EVENINGS

MUST BE FLEXIBLE AND WILLING TO WORK ADDITIONAL HRS IF NEEDED

MUST BE ABLE TO FOLLOW PROCEDURES AND POLICIES OF THE AGENCY

MAY BE REQUIRED TO TAKE COVID TESTS TO ENSURE THE SAFETY OF ALL INDIVIDUALS SERVED AND EMPLOYEES
