Your Role as a Physical Therapist :

  • Evaluate, plan, and implement personalized therapy for residents with temporary or long-term physical conditions.
  • Provide treatments in line with the plan of care and modify as needed based on resident progress.
  • Educate and support patients, families, and caregivers throughout the rehab journey.
  • Monitor and document patient responses, progress, and outcomes using the Net Health electronic documentation system (preferred).
  • Collaborate with our multidisciplinary team to promote residents’ healing, independence, and overall quality of life.

    • What We’re Looking For :

  • Current Physical Therapist license in the State of Pennsylvania (in good standing).
  • SNF / Long-Term Care experience preferred.
  • Strong communication and interpersonal skills.
  • Familiarity with modern PT practices and documentation systems.
  • Compassionate, patient-centered mindset.
