As the Clinical Coordinator, you will advance the company’s mission of providing quality service by facilitating and providing case management and counseling services.

• Provide case management and counseling services to an assigned individual caseload. Assess referred individuals and lead development, coordination, and evaluation of service plan in conjunction with treatment team, funding source, and key stakeholders.
• Maintain current and complete records in accordance with corporate, contractual, and regulatory standards.
• Provide ongoing monitoring and consultation to mentors regarding implementation of service plan, including pre-service introduction and continuing education.

For caseloads that include mentor individuals, make mentor and family home visits, and conduct mentor group meetings as required.
