Job Description

The Nurse Practitioner role offers primary care services to elderly participants through an interdisciplinary team, providing preventive, acute, and long-term care onsite and in the community.

What You'll Do :

Provide supervised primary medical care, conduct exams, diagnose, and treat illnesses

Collaborate with the care team for 24-hour wraparound care

Participate in care planning, documentation, and communication

Ideal Candidate :

Master’s in Nursing, current RN & NP licensure in Michigan

At least 1 year experience with elderly or frail adults

Strong communication skills, proficiency in EMR and MS Office

Reliable transportation & willingness to work in varied environments

Must meet vaccination and competency standards, and be flexible with hours and on-call duties
