Key Responsibilities

Patient-Centered Care and Clinical Excellence

  • Patient Assessment : Perform comprehensive assessments during each visit, monitoring vital signs, wound care, and medication administration in accordance with care plans.
  • Care Plan Implementation : Deliver nursing care as outlined in the patients individualized care plan, ensuring accuracy, quality, and compliance with regulatory standards.
  • Patient Education : Provide patients and families with education on managing health conditions, proper medication use, and self-care strategies to promote independence and understanding.

Operational Excellence

  • Coordination of Care : Collaborate with registered nurses, physicians, and other care team members to ensure seamless care delivery and continuity of services.
  • Documentation : Maintain accurate and timely documentation of care activities, including assessments, medication administration, and changes in patient condition.
  • Performance Metrics : Ensure productivity by completing all assigned visits and meeting performance expectations while maintaining high-quality care delivery.

  • Education : Current and valid Licensed Practical Nurse (LPN) license issued by the state. Completion of an accredited nursing program.
  • Experience : At least one year of clinical nursing experience in a home health or healthcare setting, with knowledge of administering care in patients' homes.
  • Clinical Competency : Demonstrated ability to perform skilled nursing tasks, including medication administration, wound care, monitoring vital signs, and assisting with mobility and activities of daily living.
  • Regulatory Knowledge : Familiarity with state and federal home health regulations, including HIPAA and CMSs Conditions of Participation, to ensure compliance in patient care.
  • Communication Skills : Excellent interpersonal and verbal communication skills to build trust and maintain effective relationships with patients, families, and interdisciplinary care teams.
  • Compassion and Empathy : A commitment to providing patient-centered, compassionate care that respects the dignity and independence of each patient.
