Overview : The Home Health RN will be handling roughly 30 patient cases where they will travel to patient homes (5-7 per day) to do in home assessments, wound care, medication administration and refills, patient education and helping to create a nursing care plan.
Responsibilities :
Completes initial evaluation visits, including history and physical exam.
Regularly evaluates the patient's nursing needs.
Develops nursing care plan and makes necessary revisions.
Provides health services requiring substantial specialized nursing skills including appropriate preventative and rehabilitative nursing procedures.
Utilizes other community agencies and resources to provide comprehensive patient care.
Coordinates additional health services provided to patient.
Discuss changes in the patients' condition and needs with the physician and other appropriate personnel.
Supports the patients' family in meeting the physical, psychosocial and other health needs of the patient.
Oversees work of Home Health Aides as required in the patients home.
Provides community consultation and health education when appropriate.
Keeps accurate records of patients progress.
Has overall responsibility for maintaining their patients records according to Home Health Services protocol.
Participates in the regular case review meetings.
Participates in the weekend / holiday rotation.
Must provide a minimum of 25-30 patient encounters per week for a full-time position.
Requirements :
- 1+ years of experience as a home health, PACE, long term care or other skilled long term nursing background
- Have private transportation
- MA RN License
- Able to work independently
