Job descriptionTerritory includes, Houston and surrounding areas Provide individual and family counseling in home Complete relevant documentation Consult with hospice team (IDT) Participate in IDT Develop care plans Provide resources to assist patient and families in the community Provide PASSR information and coordinate respite care Application assistance to Medicaid and other insurance providers Provide case management, advocacy, resource linkage as needed Provide visits as directed to include admission, recertification, and care plan necessitated Maintain professional relationship with staff and referral sources Master's degree in social work and associated licensure - MSW 1 year experience preferred working as social worker with hospice or home health care. Ability to work in a professional manner Must work well both independently and in a group setting
