Travel RN Case Manager – LTAC

Location : Hummelstown, PA

Schedule : Days | 5x8s

Assignment Length : 13 Weeks

HumanEdge Health is seeking an experienced Travel RN Case Manager for a 13-week assignment at a Long-Term Acute Care (LTAC) facility in Hummelstown, PA . The Case Manager plays a vital role in managing patient care plans, ensuring quality outcomes, and coordinating safe, efficient discharges.

Key Responsibilities :

Conduct utilization reviews, discharge planning, and treatment coordination

Develop and manage individualized clinical plans of care

Collaborate with the interdisciplinary team and physicians to resolve barriers to care

Educate and involve patients and families in discharge and treatment plans

Complete documentation accurately and in a timely manner

Identify and refer patients to appropriate community resources

Perform medical necessity reviews for admissions and continued stays

Communicate with insurance providers to ensure timely authorizations and approvals

Maintain awareness of payor requirements and regulatory standards

Review billing practices to support reimbursement and minimize financial risk

Assist with data collection, quality metrics, and internal reporting

Attend case conferences and collaborate on care coordination

Step into Director of Case Management duties as needed

Requirements :

Active RN license in Pennsylvania or compact state

Minimum of 2 years of acute care or LTAC case management experience preferred

BLS certification required

Strong knowledge of discharge planning, resource management, and payor guidelines

Ability to work independently while collaborating with the broader clinical team

Excellent communication, organizational, and problem-solving skills
