Social Work- Pediatric
Day Shift
Full Time, 32 hrs. per week
Potential for on- call every 4th weekend
1 -2 holidays / year
Functions :
Assesses patient / family social and emotional factors to determine ability to cope with serious illness and death.
Identifies support systems available to reduce stress and facilitate coping with chronic, acute, and end-of-life care.
Identifies the developmental needs of patient / family and obstacles to learning or ability to participate in care of patient; assesses caregiver's ability to function.
Identifies obstacles to adherence and assists in patient / family / caregiver understanding of goals and interventions; address patient / family / caregiver questions and issues.
Assesses for potential risk for suicide and / or abuse or neglect.
Assesses the environmental resources and obstacles to maintaining patient safety.
Identifies family dynamics and communication patterns; assess for special needs related to cultural diversity including communication, space, role of family members and special traditions.
Assists in addressing / supporting decision-making, ensuring the best interest and rights of the child and withstanding complex, competing, and shifting goals of care.
Educates and evaluates patient / family understanding and acceptance to change in program / level of care; communicate psychosocial information to include goals of care to new care providers, to include hospital or Hospice inpatient facility when transfer to a different care setting.
Assists with Advance Care Planning including funeral planning.
Assess for existing plans, assist patient and family in exploring options and planning for time of death.
Collaborates with Bereavement Counselors to provide support and resources for classmates and teachers of children in Sunflower Pediatrics program, and their siblings when appropriate and necessary permissions have been obtained : provide in-school visits, to include IEP meetings with school personnel; provide pre- and post-death classroom visits when appropriate and requested by the parent and approved by the school.
Provides clinical therapeutic services to pediatric patients and families such as, but not limited to, individual counseling (loss, anticipatory grief and bereavement, coping, transition) to patient and / or family, crisis intervention, sibling support (in collaboration with Bereavement Counselors), longer-term individual and family counseling for pediatric palliative care patients if indicated.
Establishes treatment goals and revises counseling interventions as indicated. Evaluate patient / family response to psychosocial interventions; identify obstacles to illness management and assist in understanding goals of intervention.
Consults with primary care and other providers to provide information about the full range of clinical resources available to the child; identifies and makes referrals to other community resources and services to support the plan of care and needs of the child and family and assesses patient / family / caregiver ability to access them.
Evaluates patient / family / caregiver response to intervention(s) when referred to community agency and the satisfaction of the services(s) provided, in addition to providing supportive presence for those staff (private duty nursing staff, etc).
Collaborates with primary care and other providers to support and promote the normal growth and development of the child and his or her siblings.
Participates in meetings as a contributing member of the team in ongoing communication, coordination, and continuity of the patient and family care / service needs from the psychosocial perspective, this includes developing and revising patients POC.
Provides social service coverage after hours, as needed.
Participates in program, organizational, community in-service programs or presentations, as appropriate, to support palliative care and hospice education and promote awareness of the Sunflower pediatric program.
Participates in organization education.
Ensures timely completion of all discipline-specific and organizational documentation according to policies and procedures.
Maintains a calm, non-anxious presence, employing deep listening, non-judgment and silence as appropriate, in order to form supportive, connective bonds, while preserving professional boundaries.
Communicates effectively with organizational staff, physicians, patients, families, nursing home staff, outside contractors, community agencies.
Works independently and effectively in a variety of settings including but not limited to private homes, long-term care facilities, hospitals, outpatient clinics, and group homes.
Maintains a safe environment by utilizing and adhering to safe work policies and practices.
Attends all mandatory meetings and educational programs and participates in Quality Assurance and Performance Improvement (QAPI) activities as assigned.
Completes at least 1 hour of pediatric hospice and palliative care specific education per year.
Assists with the orientation of new staff, visiting professionals, students, etc.
Qualifications : MSW required
LSW required within 1 year of hire date
Experience with medically fragile children and families
