RN Case Manager PRN

Job Summary

Pay : $40.15$58.58 per hour.

We are seeking an RN Case Manager PRN to coordinate clinical and psychosocial care across the continuum of patient care. The role supports interdisciplinary teams to ensure efficient, cost?effective utilization of resources, assess admission appropriateness, plan and execute timely discharge, and monitor Quality & Risk Management issues.

Responsibilities

• Manage patient case load through interdisciplinary collaboration.
• Conduct assessments to determine admission and care level.
• Initiate discharge planning and update it continuously during hospitalization.
• Monitor and report Quality & Risk Management concerns to the QRM department as part of the CQI process.
