We are looking for a Social Services Case Manager to join our team! We are hiring in or near Lawrenceville, GA.

How You'll Make a Difference :

The position ofSocial Services Case Managerwill provide support and social services to health plan members and their families in accordance with an appropriate plan of care. The Social Services Case Manager provides in-home and telephonic assessment of needs, while continuing to develop, monitor, and evaluate care plans and progress, while facilitating interdisciplinary approaches to social needs.
