American Traveler is hiring an experienced RN Case Manager for a Home Health position in Wisconsin, requiring home health experience and a WI or compact RN license.
\n \n Responsibilities\n
Home Health setting serving patients of all ages in their homesResponsible for case management including assessment, care planning, coordination, and evaluation across the health care continuum12-hour day shiftsTravel to patient residences using personal vehicleMay be required to float to other facilities in the Central Region including Wausau, Merrill, and LangladeResponsible for developing, implementing, evaluating, and revising individual care plans in partnership with an interdisciplinary teamProvides direct patient care, supervises Home Health Aides and LPNs, and coordinates resources for optimal outcomesParticipates in on-call rotation and may cover sick or vacation shifts as neededUses best practices for pain and symptom management and acts as patient advocate with physiciansDocuments all assessments, care, and communication promptly according to established standardsExpected to participate in interdisciplinary meetings and quality improvement initiativesAnnual flu and other vaccinations may be required by agency policyTravelers may on occasion be called off up to 36 hours per 13-week contract\n \n Requirements\n
Active Wisconsin or compact RN license requiredCurrent BLS certification from AHA or Red Cross required or obtained within 90 days of hireMinimum five years of broad-based nursing experience preferredPrevious home health or hospice nursing experience preferredValid driver’s license (Wisconsin or Michigan) and automobile liability insurance with at least $100,000 / $300,000 coverage requiredAbility to travel to client homes for care deliveryDiploma, Associate, or Bachelor's Degree in Nursing\n Benefits\n
