Family Practice - Geriatrics Physician

Primary Care Provider - Kokomo, IN / Jeffersonville, IN

Job Type : Full-Time

Shifts : Days (Monday-Friday); Shared on-call coverage (no weekend shift differential)

Healthcare Setting : Outpatient Senior Care / Community Health

Responsibilities :

Provide direct primary care services, including assessments, diagnoses, treatment, and medical record documentation

Collaborate with interdisciplinary care teams to coordinate patient-centered care

Offer medical consultations and support to clinical staff and care coordinators

Participate in care planning meetings and contribute to clinic operations

Deliver age-specific and geriatric care with a focus on safety and dignity

Ensure compliance with clinical protocols, safety standards, and regulatory guidelines

Maintain accurate and timely documentation in the electronic health record (EHR)

Attend staff meetings, trainings, and internal committees as required

Qualifications :

MD / DO degree from an accredited medical school

Board Certification in Family Medicine or Internal Medicine

Licensed or eligible to practice in Indiana

CPR certification preferred

At least 2 years of experience working with frail or elderly populations in long-term care

Experience in senior care or PACE programs is a plus
