PT- home health needed asapPlans and conducts segments of treatment programs designed to restore and improve physical, social and mental functions, while meeting department objectives. Serves as co-leader in interdisciplinary approach to patient groups.Bachelor's Degree or equivalent (some certifications will require Bachelor?s) : RequiredMaster's Degree : PreferredPhysical Therapy licensure in all applicable states of practice : RequiredCardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS OR HS-BLS OR RQIBLS) certification : RequiredValid Driver's License (DL) and must be at least 21 yrs of age or older : Required
