Your Role Will Include: Comprehensive Patient Care: - Conduct physical exams, assess symptoms, and diagnose medical conditions
- Order and interpret diagnostic tests, including blood work, imaging, and other necessary screenings
- Develop and implement individualized treatment plans based on patients' needs and conditions
- Prescribe medications, therapies, and interventions to manage acute and chronic health conditions
- Provide ongoing management for both acute and chronic diseases, ensuring patients' health is properly monitored
Patient Education & Support: - Educate patients and their families about health conditions, treatment options, and lifestyle changes for improved well-being
- Offer preventive care services, including immunizations, screenings, and wellness check-ups
- Guide patients on self-care, nutrition, and long-term health maintenance strategies to promote healthier lifestyles
Collaboration with Healthcare Teams: - Work closely with physicians, specialists, and other healthcare professionals to coordinate and deliver patient care
- Participate in case discussions and share insights to develop effective patient management plans
- Provide mentorship and guidance to other members of the healthcare team, as needed
Ongoing Monitoring & Adjustments: - Regularly assess and monitor patients' responses to treatments, adjusting care plans as necessary
- Ensure comprehensive documentation of all patient interactions and treatment plans in electronic health records (EHR)
- Review patient feedback and actively seek improvements in the quality of care provided
Qualifications: - Master's degree in nursing from an accredited Nurse Practitioner program
- Active Washington Nurse Practitioner license
- Board Certification as a Nurse Practitioner (e.g., Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner, etc.)
- Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications preferred
- Experience in a Nurse Practitioner or Registered Nurse (RN) role is preferred
- Strong clinical skills to assess, diagnose, and manage a wide variety of health conditions
- Excellent communication skills for patient interaction and teamwork
- Ability to make independent clinical decisions while maintaining collaboration with a healthcare team
