Job Summary

Provide direct patient care as initiated by the registered nurse

Assess and provide patient and family / caregiver education and information pertinent to diagnosis and plan of care

Report to supervisor on all patients under your care.

Provide accurate and timely documentation

Participates in coordination of home health services with team

Performs other duties as assigned by the registered nurse

Qualifications

Currently a Licensed Vocational Nurse (LVN) in state of employment

Previous experience as a Licensed Vocational Nurse is preferred

Excellent interpersonal and client care skills

Valid driver’s license

First aid / CPR certification required
