Family Practice - Geriatrics Physician

You will be joining one of the largest nonprofit health systems in the state of Florida. The current need is for a full-time Primary Care / Geriatrician to join our Skilled Nursing Facility team. The team provides physician coverage across sites of care, to ensure safe transitions, and aid in the appropriate placement of patients in Skilled Nursing Homes, Assisted living facilities, Independent living facilities or to home with support services. We are seeking physicians with strong interpersonal and leadership skills, with a demonstrated understanding of effective care planning and management for improved quality outcomes. This is a critical service line playing a pivotal role in reducing readmission rates and supporting safe patient care planning. We need caring people who care for people!
