Paragone Solutions is seeking a Non-Clinical Case Manager to support medical readiness and deployment efforts for the Kansas Army National Guard. This role focuses on maintaining the individual medical and dental readiness of ARNG personnel.

The Case Manager will collaborate closely with the Care Coordinator, who provides non-clinical administrative support. Together, they will conduct weekly reviews of assigned cases to ensure that all aspects of the case management process—assessment, planning, implementation, coordination, monitoring, and evaluation—are effectively addressing the health and human service needs of soldiers.

This position requires expertise in applying specialized knowledge and skills to coordinate and manage care, provide guidance, and promote health education for individuals experiencing changes in their normal health processes or requiring support in maintaining wellness and preventing illness, injury, or disability. Non-clinical case management personnel must be experienced and fully qualified within their respective functional areas.

Responsibilities – The Case Manager will :

  • Track the medical status of all assigned Soldiers from the identification of the non-deployable condition (T3, T4, P3 or P4 profiles) to its final determination - the Soldier is deployable, the Soldier enters the Integrated Disability Evaluation System, or when he or she is administratively separated from the military.
  • Read, understand and execute DoD, DA, NG and KSARNG regulations and policies through the application of duties.
  • Demonstrate a knowledge and proficiency in the use of several DOD web-based electronic data systems; (e.g. Medical Operational Data System (MODS) and Medical Electronic Data for Care History and Readiness Tracking (MEDCHART), MEDPROS Web Data Entry (MWDE)).
  • Scanning and filing of digital and physical Soldier Treatment Records.
  • Initiate and document physical profile and follow up on treatment requirements.
  • Log contact (email, telephone call, or personal interview) and updates with SM / cases in appropriate medical tracking systems.
  • Initiate, process and track medical board cases through applicable electronic data system.
  • Attend Periodic Health Assessment (PHA) events as determined by the needs of the KSARNG and the individual units.
  • Ensure SM’s referred by a provider are followed up by the case management team.
  • Contact SM and physician / dental offices to obtain medical documentation and information regarding open cases for treatment plan review.
  • Support SM in becoming Fully Medically Ready (FMR) by tracking medical and dental appointments.
  • Communicate with units regarding SM’s medical appointments to ensure attendance and submission of follow-up documentation.
  • Request profile, behavioral health exams / evaluations and physical exams at Periodic Health Assessment (PHA).
  • Review and track health assessment lab results, contacting the unit and SM to follow-up if abnormalities are found or coordinate if further testing is required.
  • Document cases in the eCase module of MEDCHART within one business day of notification of appointment, lab results, or action related to the treatment plan.
  • Coordinate with SMs who have LOD and non-LOD injuries or illness to ensure that the care is property managed, minimizing non-deployable status.
  • Initiate, process and track medical board cases through the Disability Evaluation System (DES) or Integrated Disability Evaluation System (IDES).
  • Maintain SM medical records in accordance with Army Regulation (AR) 40-66, The Privacy Act of 1974 and HIPAA standards and guidelines.
  • Coordinate with medical providers on care and communicate updates of the SM’s status and furnish them with the mandatory information and documentation. Submit case processing, medical documentation, and profile concerns to military or contacted provider.
  • Appropriately label, file and / or destroy medical documents after uploading into the appropriate Electronic Health Records system (EHR).
  • Participate in periodic meetings to review all cases in progress as needed to meet timelines for processing as established by various regulations, policies or KSARNG directorates.
  • Prepare and submit monthly reports documenting status of cases under case management.

Requirements :

  • Education / Experience - a minimum of 4 years of experience in either :
  • a. Medical or laboratory clinic administration or management; or

    b. Medical and / or non-medical care coordinator experience; or

    c. Clinic or laboratory support services

  • Security - Must be able to pass a national security background check for access into military electronic systems.
  • Must maintain active and valid HIPAA certification
