• Setting : HYBRID (Home Visits & Virtual)
• Location : San Antonio, TX
• Schedule : Mon-Fri (8a-5p)
• Duration : Permanent
• Duties :
• Core NP (Homecare) : Travel to patients homes
• Conducting assessments both in patients homes and in virtual environment
• Counsel and educate patients and families about benefits and programs available.
