Efficiency : We utilize tools to make our workflow productive. Obtains medical referrals as needed per insurance guidelines. Utilize appropriate educational materials, meal planning techniques, and technology including digital meal planning and / or food tracking software to help clients achieve their goals. Confirms patient paperwork is complete at least 24 hours before the appointment and responds by taking appropriate action. Provides medical nutrition therapy (MNT) to patients in person at our 2615 Genesee Street, Utica, NY location, and via telehealth, following the Academy of Nutrition and Dietetics Nutrition Care Process (NCP). Collects, classifies, and synthesizes important and relevant data to identify nutrition-related problems and their causes. The RDN will identify whether previously collected information has changed or remained the same for nutrition reassessment. Identifies the nutrition problem, determines the etiology, and documents the corresponding signs and symptoms using critical thinking skills to identify and prioritize problems that can be resolved or improved by the RDN. Plans and implements purposeful actions to change or improve a nutrition-related problem. Determines and measures the progress made for the nutrition intervention and whether the nutrition-related goals or expected outcomes are being met. Document findings, actions, summary assessments, follow-up and diet instructions, education, and comprehension in the practice's EHR within 24 hours of the patient's service date. Documentation is consistent with The Academy of Nutrition and Dietetics standards. As medically appropriate, make appropriate referrals to other services, such as therapists, Social Services, or various Food Assistance Programs. Schedules and reschedules nutrition counseling appointments in the practice's EHR (Practice Better) with a cadence that supports the medical necessity of the patient's condition. Participates in Bloom Nutrition Solutions in-services and meets the mandatory annual in-service requirements established by the practice. Actively participates in monthly staff meetings. Assists in projects to increase patient referrals as needed and as time permits. Participates in community health care events as time permits. Other duties as assigned.
Job Type : Part-time, Per diem, employee
Salary : $30.00 - $38.00 per hour with potential for additional "non-clinical" based roles at varying rates and / or stipends.
QUALIFICATIONS
- Remote position - Telehealth experience (Preferred)
- Active Certification with NYSED as a Dietitian-Nutritionist in the state of New York.
- Experience working as an outpatient dietitian. (Prefered)
SCHEDULE REQUIREMENTS
This part-time, On site position is scheduled for 4-10 hours per week as needed with potential to grow into part time hours of 20 or more per week depending on the need. Looking for Monday, Saturday, and / or evening or early morning availability i n-person at our Utica, NY office.
