Schedule : Monday to Friday, 9 : 00 AM – 5 : 00 PM (non-flexible)

Position Summary
We are seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to perform structured wellness and clinical assessments, document findings, coordinate care with the internal clinical team and external partners, and participate in community wellness events. This role involves traveling to patients’ homes and is ideal for an experienced LPN who is comfortable working independently in the community.
Key Responsibilities
- Conduct clinical assessments for patients participating in care management programs.
- Obtain and record vital signs, including blood pressure, height, weight, and pulse.
- Perform medication reconciliation and basic physical assessments.
- Accurately document medical, social, and family history in the electronic medical record (EMR).
- Deliver health education and promote patient engagement to support adherence and self-management.
- Ensure all clinical documentation is completed accurately, thoroughly, and within established timeframes.
- Communicate effectively with the Director of Nursing and administrative staff regarding visit outcomes, patient needs, and any required follow-up.
- Collaborate with outreach and clinical teams to support Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) workflows.
- Represent the organization at community health and wellness events as needed.
- Provide professional, compassionate, and culturally sensitive care in both home and telehealth settings.
- Maintain compliance with HIPAA regulations and all applicable state and federal healthcare standards.
Qualifications
Active Licensed Practical Nurse (LPN) license in the state of Georgia — requiredMinimum of 5 years of clinical experience; prior experience in care coordination or case management strongly preferredDemonstrated excellence in patient care and customer serviceStrong communication, documentation, and interpersonal skillsStrong critical thinking and problem-solving skillsProficiency with electronic medical records (EMRs) and comfort using technology for charting and workflow managementReliable transportation and willingness to travel throughout Metro Atlanta and surrounding areasAbility to work independently while maintaining a high level of professionalism, integrity, and discretion
